37-10 February 5, 2010 RESOLUTION WHEREAS, the Township of West Orange, Essex County, State of New Jersey has solicited informal quotations in conjunction with the proposed selection of a medical and prescription insurance provider; and WHEREAS, the Township of West Orange reviewed the proposals and the guarantee of benefits remaining the same as under the State Health Benefits Program; and WHEREAS, the Township of West Orange reviewed the proposals; and WHEREAS, the contract for insurance constitutes an enumerated exception under N.J.S.A. 40A:11-5(1)(m) of the Local Public Contracts Law; and WHEREAS, termination of participation in the New Jersey State Health Benefits Program is allowed under N.J.S.A. 52:14-17.25 et. seq.; and WHEREAS, the Township of West Orange believes that the best proposal submitted is from Horizon Blue Cross Blue Shield of New Jersey. NOW, BE IT HEREBY RESOLVED, this 5th day of February by the Township of West Orange that the Township of West Orange selects Horizon Blue Cross Blue Shield of New Jersey as its medical insurance provider effective May 1, 2010 in accordance with the proposal submitted by Horizon Blue Cross Blue Shield of New Jersey dated December 10, 2009; and it is further RESOLVED, that the Township of West Orange authorizes and directs the Business Administrator to execute any and all necessary documents in order to implement the intent of this resolution; and it is further RESOLVED, that the Township of West Orange recognizes Fairview Insurance Associates Agency as Broker of Record for the Horizon Blue Cross Blue Shield of New Jersey medical plans.
