Plan-Aflac 10-26-10.doc 277-10 October 26, 2010 RESOLUTION WHEREAS, the Township of West Orange (“West Orange”) desires to establish a Medical Care Expense Reimbursement Plan and/or a Dependent Care Expense Reimbursement Plan for its employees; and WHEREAS, West Orange desires to have American Family Life Assurance Company of Columbus (“AFLAC”) serve as the Plan Administrator for the program; and WHEREAS, annexed hereto is a copy of the standard Reimbursement Services Agreement to be signed by both West Orange and AFLAC; NOW THEREFORE BE IT RESOLVED by the Township Council of the Township of West Orange that the Mayor be and hereby is authorized to execute and the Township Clerk be and hereby is authorized to attest to the annexed Reimbursement Services Agreement. ________________________________ Karen J.
