10-16 VETERINARY SUPERVISION SERVICES 2016 – 2017 AGREEMENT THIS AGREEMENT entered into this 1st day of January, 2016 between the Township of West Orange, 66 Main Street, West Orange, NJ 07052 (the “Township”), and Robert DeFilippis, D.V.M. Roseland Animal Care, 215 Eagle Rock Avenue, Roseland, NJ 07068 (“Dr. DeFilippis”), for veterinary supervision at the Alex Caprio Animal Control Facility, 311 Watchung Avenue, West Orange, NJ 07052 (“ACACF”). 1. As required pursuant to N.J.A.C. 8:23A-1.9, Dr. DeFilippis, a licensed Veterinarian (02194) by the State of New Jersey, agrees to provide veterinary supervision of the ACACF for the period of January 1, 2016 through December 31, 2017. 2. In his role as Veterinarian, Dr. DeFilippis shall work with the Township Health Officer to establish a program to maintain the control of insects, ectoparasites, avian and mammalian pests at the ACACF as part of the overall disease control program. 3. In his role as Veterinarian, Dr. DeFilippis shall provide such supervision on a weekly basis to inspect animals held at this facility and provide basic veterinary care when required and ensure that an adequate program for the control of disease is maintained and adhered to at the ACACF. 4. In his role as Veterinarian, Dr. DeFilippis shall provide and certify Township Animal Control personnel in the approved euthanasia techniques used at the facility. 5. In this role as Veterinarian, Dr. DeFilippis shall from time to time provide a licensed Veterinarian in his absence. 6. The Township agrees to pay Dr. DeFilippis the sum of $8,406.00, payable in quarterly installments of $2,101.50, for the period of January 1, 2016 – December 31, 2016 and the sum of $8,532.00, payable in quarterly installments of $2,133.00 for the period of January 1, 2017 – December 31, 2017.
