VETERINARY SUPERVISION SERVICES 2018 — 2019 AGREEMENT THIS AGREEMENT entered into this _/s* day of January 2018 between the Township of West Orange, 66 Main Street, West Orange, NJ 07052 and Robert DeFilippis, D.V.M. Roseland Animal Care, 215 Eagle Rock Avenue, Roseland, NJ 07068 for veterinary supervision at the Alex Caprio Animal Control Facility (ACACF) 311 Watchung Avenue, West Orange, NJ 07052. 1. As required in N.J.A.C. 8:23A-1.9, Robert DeFilippis, D.V.M., a licensed Veterinarian (02194) by the State of New Jersey agrees to provide veterinary supervision of the Alex Caprio Animal Control Facilty for the period of January 1, 2018 through December 31, 2019. 2. In his rote as Veterinarian, Robert DeFilippis shall work with the Health Officer of the Township of West Orange to establish a program to maintain the control of insects, estoparasites, avian and mammalian pests at the ACACF as part of the overall disease contro! program. 3. In his role as Veterinarian, Robert DeFilippis shal! provide such supervision on a weekly basis to inspect animals held at this facility and provide basic veterinary care when required and ensure that an adequate program for the control of disease is maintained and adhered to at the ACACF. 4. In his role as Veterinarian, Robert DeFilippis shall provide and certify West Orange Township Animal Control personnel in the approved euthanasia techniques used at the facility. 5. In this role as Veterinarian, Robert DeFilippis shall from time to time provide a licensed Veterinarian in his absence. 6. The Township of West Orange agrees to pay Robert DeFilippis the sum of $8,532.00 payable in quarterly installments of $2,133.00 for the period of January 1, 2018 - December 31, 2018 and the sum of $8,532.00 payable in quarterly installments of $2,133.00 for the period of January 1, 2019 ~ December 31, 2019.
