1 167-18 July 17, 2018 RESOLUTION WHEREAS, the Essex County Division of Senior Services (the “County”) has agreed to provide partial funding for a Senior Citizen Public Health Nurse program (the “Nurse Program”) for the contract period(s) of: (i) January 1, 2019 through December 31, 2019, in the amount of $16,127; (ii) January 1, 2020 through December 31, 2020, in the amount of $16,127; and (iii) January 1, 2021 through December 31, 2021 in the amount of $16,127; and WHEREAS, the County has provided a proposed agreement for the partial funding incorporating the terms and requirements in the County’s Request for Proposal, annexed hereto as Exhibit “A;” WHEREAS, the County also requires the Township to adopt its Harassment, Discrimination, Retaliation Prevention Policy and Complaint Procedure Policy (the “Discrimination Policy”), annexed hereto as Exhibit “B,” for the Nurse Program; and WHEREAS, the County also requires the Township to adopt its Sexual Harassment Policy, annexed hereto as Exhibit “C,” for the Nurse Program; and WHEREAS, the Township’s Health Officer has reviewed the annexed agreement and recommends execution and approval of the contract; and WHEREAS, the Township has participated in this program in previous years and the Nurse Program provides a great benefit to the residents of the Township; and WHEREAS, the Township agrees that the Discrimination Policy and Sexual Harassment Policy are appropriate for the Nurse Program. NOW, BE IT HEREBY RESOLVED, by the Township Council of the Township of West Orange that the Mayor be and is hereby authorized to execute the annexed agreement with the County, annexed hereto as Exhibit “A” for the time periods specified herein, in connection with the partial funding of the Nurse Program; and be it further

2 RESOLVED that the Municipal Clerk shall be and hereby is authorized to attest to the Mayor’s signature; and it is further RESOLVED, that a permanent copy of this agreement shall be maintained by the Municipal Clerk and the Department of Health; and it is further RESOLVED that the Township adopts the County’s Discrimination Policy, annexed hereto as Exhibit “B” for the Nurse Program; and it is further RESOLVED that the Township adopts the County’s Sexual Harassment Policy, annexed hereto as Exhibit “C” for the Nurse Program.
