SLACK ENVIRONMENTAL SERVICES, INC. (TSES) is pleased to submit the following proposal for your review. The scope of work is in accordance with existing New Jersey department of Environmental Protection (NJDEP) Bureau of Underground Storage Tanks (BUST) Standards. Scope of work is based on the information provided to TSES by the client. TSES therefore assumes the following: A. There were no discharges of free product. B. Soils excavated are below current NIDEP Cleanup Criteria. C. The UST’s to be removed should have been registered with the NJDEP since 1988. D. Retain a Licensed Site Remediation Professional (LSRP) for UST Closure Site Investigation Report, and Response Action Outcome Letter. Any variation from the above-stated facts will result in re-evaluation of costs stated herein. Customer authorizes TSES to perform any and all additional services or tests required by NJDEP not specified herein at an additional cost for labor and material. Pricing is from our NJ State Contract #A42266. REMOVAL SCOPE OF WORK: 1 The UST’s must be registered with the NJDEP in order to remove them (if not previously registered), If the UST(s) have not been registered, a UST Facility Certification Questionnaire, a site map, and a check payable to “Treasurer State of New Jersey” will have to be submitted tothe D.E.P. The NJDEP will then issue a Facility Registration number / Facility ID# for the application submitted. Once TSES receives the approved NIDEP “Closure of an Underground Storage Tank System” form, we can then notify the local municipal agency and building department of the closure activity in writing and apply for necessary permits, A copy of the approved NJDEP closure form will be submitted with the permit applications to the local building department. Once the building department approves the permit applications, TSES can then commence work, 3. TSES will call the NJ ONE CALL Hotline to mark out all the underground utilities in the excavation area. This will be done at least FIVE days prior to excavation. T. Slack cannot begin excavation until all the proper utility lines are clearly marked out. 4. The UST(s) contents will be pumped dry prior to any excavation. (See Contingency Costs). 5. We will excavate to the top of UST(s), drain all lines back into tank and purge tank. Once purged, a combustible gas meter will be used to determine if the level of combustible gas in the UST(s) is within safe limits to cold cut and clean to Fire Department and Department of Transportation Standards. All liquids and sediment will be containerized for disposal and manifested according to Federal and State Guidelines. 6. Once cleaned, the UST(s) will be removed from excavated area following API 1604 Procedures. 7. All post-excavation confirmatory sampling will be performed by a Licensed Site Remediation Professional (LSRP) from TSES certified by the NIDEP in Subsurface Evaluation. The sampling and analysis will be performed in accordance with the NIDEP’s Technical Guidance Document N.J.A.C. 7:26E. 8. TSES will arrange for all necessary municipality inspections. 9. The cleaned UST(s) will then be cut, transported and disposed/scrapped. All scrap-yard receipts to be provided to the client as proof of disposal. 10. The excavation areas will be backfilled with certified clean fill compacted in 24" lifts to grade. 11. We will restore the disturbed area as required up to grade only. 12, TSES will fill out the UST Facility Certification Questionnaire form to de-register the tanks removed. The completed questionnaire will be sent to the client for original signature by the responsible party. The client MUST submit the signed questionnaire to the NJDEP. This

This form must be signed, completed, and submitied to the NIDEP WITHIN 7 DAYS OF THE UST REMOVAL. This is the client’s responsibility; T. Slack cannot sign off on this questionnaire. 13. TSES will prepare and submit a full Site Investigation Report (SIR) and or a Remedial Investigation Report (RIR) as required by the NJDEP. The complete report with electronic data will be sent to the client for original signature by the responsible party. IT IS THE CLIENT’S RESPONSIBILITY TO SUBMIT THE REPORT TO THE NJDEP. Failure to do so will result in possible penalties issued by the NIDEP (See review fee below).
