A completed Property Owner Certification Form. The NJDEP Public Notice Form and the following: Verification that legal notification has been sent to the Township of West Orange Planning Board, Environmental Commission and Construction Official, the Essex County Planning Board, Hudson-Essex-Passaic Soil Conservation District, and certified lists of property owners within 200 feet of the legal boundary line. The legal notices for this Flood Hazard Area Individual Permit resubmittal and separate LOI extension have been provided in the same certified mail envelopes. A digitally signed and sealed copy of the Regulated Water + Riparian Zone Impacts Plans. Annotated color photographs showing the portion of the site that will be disturbed by the proposed activities. Photo Location Map that shows the locations and directions of the photographs. A copy of the Flood Hazard Area Environmental Report, which includes the following: a. A Statement of compliance describing the project and proposed activities as well as a list and description of compliance of all limits and requirements at N.J.A.C. 7:13-11.1, 11.2, 11.5, and 11.6. b. A copy of the current municipal tax maps, appropriate portion of the USGS Caldwell and Orange, New Jersey quadrangles, local street map, Landscape Project (Version 3.3) habitat mapping, and FEMA mapping with the site clearly indicated. A digitally signed and sealed copy of the Flood Hazard Area Engineering Report prepared by Anderson Consulting Services LLC. A copy of the response from the Natural Heritage Program regarding threatened and endangered species.
