Enclosed are: 1. Form PH-2Z, Notice of Public Hearing to applicant 2. Form PH-3Z, Notice of Hearing 3. Form PH-4Z, Affidavit of Proof of Service 4. A list of utility companies to be notified. Form PH-3Z must be submitted to the West Orange Chronicle so that your legal notice will appear at least 10 days prior to the hearing. Since the West Orange Chronicle is published only on Thursday, your legal notice must be submitted by 11:00 A.M. Friday in order to appear in the next week's edition. An affidavit of publication must be obtained from the newspaper and submitted to our office at least two (2) days before the meeting. You must mail, via certified mail, copies of the PH-3Z form to all property owners within 200 feet and utilities at least ten (10) days prior to the meeting. The white receipts must be retumed to this office along with the notarized PH-4Z form at least two (2) days prior to the meeting.
