LEPORE Director/Municipal Engineer January 3, 2018 Mayor and Township Council Municipal Building 66 Main Street West Orange, New Jersey 07052 RE: FIRE STATION NO. 4 280 PLEASANT VALLEY WAY BLOCK 152.07, LOT 1 UNDERGROUND STORAGE TANK REMOVAL & SITE REMEDIATION Mayor Parisi, Council President McCartney and Council Members: The Township of West Orange will continue to require the services of a Licensed Site Remediation Professional at Fire House No.4 at 280 Pleasant Valley Way associated with site remediation from leaking underground storage tanks that were removed in 1994 and the removal of existing underground storage tanks installed in 1994. The underground tanks will be replaced with above ground storage tanks. That work will be performed under a separate contract. The Licensed Site Remediation Professional for this site is Manal Baba of T & M Associates, 11 Tindall Road, Middletown, New Jersey 07748. Ms. Baba and T & M Associates will continue to provide these services. Attached please find their proposal dated September 29, 2017 for this additional work. There are six tasks to be performed with these services for tank removal and site remediation. Task 1, Licensed Site Remediation Professional (LSRP) $ 1,300.00 Project Management/Notice of Intent to Close Regulated Underground Storage Tanks. Task 2. UST Removal/Closure Supervision $ 7,500.00 and Soil Sampling Task 3. Monitoring Wells Abandonment of $12,750.00 (MW-1, MW-4 & MW-11) and installation of MW-4R and MW-1R Task 4.
