Article 3.10 Affirmative Action Requirement Required Affirmative Action Evidence General Requirements of P.L. 1975, c. 127: You are hereby put on notice that: Goods, Professional Service and General Service Contracts Upon Award of a Goods and Service or Professional Services contract, it is the responsibility of the Public Agency to provide the vendor with the Initial Employee Information Report, Form AA-302, if the vender doesn’t have a Certificate or Letter of Federal Approval. The Division does not supply this form to the Vendor. If the Public Agency requires a supply of forms, the Public Agency may call (609) 292-5475 and leave a detailed message on the voice mail system or submit a request to the Division of Affirmative Action forms. Forms will be mailed out to the Public Agency upon receipt of the message. If the vendor has an expired certificate, the vendor must apply directly to the Division for a Renewal Form AA-302, Employee Information Report.
