SLACK ENVIRONMENTAL SERVICES, INC. (TSES) is pleased to submit the following proposal for your review. The scope of work is in accordance with existing New Jersey department of Environmental Protection (NJDEP) Bureau of Underground Storage Tanks (BUST) Standards. Scope of work is based on the information provided to TSES by the client. TSES therefore assumes the following: A. There were no discharges of free product. B. Soils excavated are below current NIDEP Cleanup Criteria, C. The UST’s to be removed should have been registered with the NJDEP since 1988. D. Retain a Licensed Site Remediation Professional (LSRP) for UST Closure Site Investigation Report, and Response Action Outcome Letter. Any variation from the above-stated facts will result in re-evaluation of costs stated herein. Customer authorizes TSES to perform any and all additional services or tests required by NJDEP not specified herein at an additional cost for labor and material. Pricing is from our NJ State Contract #A42266. PE OF WORK: 1. The UST’s must be registered with the NIDEP in order to remove them (ifnot previously registered). If the UST(s) have not been registered, a UST Facility Certification Questionnaire, a site map, and a check payable to “Treasurer State of New Jersey” will have to be submitted tothe DEP. The NIDEP will then issue Facility Registration number / Facility ID# for the application submitted.
