ENVIRONMENTAL SERVICES Once TSES receives the approved NIDEP “Closure of an Underground Storage Tank System” form, we can then notify the local municipal agency and building department of the closure activity in writing and apply for necessary permits. A copy of the approved NJDEP closure form will be submitted with the permit applications to the local building department. Once the building department approves the permit applications, TSES can then commence work. TSES will call the NJ ONE CALL Hotline to mark out all the underground utilities in the excavation area, This will be done at least FIVE days prior to excavation. T. Slack cannot begin excavation until all the proper utility lines are clearly marked out. The UST(s) contents will be pumped dry prior to any excavation. (See Contingency Costs). We will excavate to the top of UST(s), drain all lines back into tank and purge tank. Once purged, a combustible gas meter will be used to determine if the level of combustible gas in the UST(s) is within safe limits to cold cut and clean to Fire Department and Department of Transportation Standards. All liquids and sediment will be containerized for disposal and manifested according to Federal and State Guidelines. Once cleaned, the UST(s) will be removed from excavated area following API 1604 Procedures. All post-excavation confirmatory sampling will be performed by a Licensed Site Remediation Professional (LSRP) from TSES certified by the NJDEP in Subsurfece Evaluation. The sampling and analysis will be performed in accordance with the NIDEP’s Technical Guidance Document N.J.A.C. 7:26, TSES will arrange for all necessary municipality inspections. The cleaned UST(s) will then be cut, transported and disposed/scrapped. All scrap-yard receipts to be provided to the client as proof of disposal. The excavation areas will be backfilled with certified clean fill compacted in 24" lifts to grade. We will restore the disturbed area as required up to grade only. TSES will fill out the UST Facility Certification Questionnaire form to de-register the tanks removed. The completed questionnaire will be sent to the client for original signature by the responsible party. The client MUST submit the signed questionnaire to the NIDEP. This form must be signed,

This form must be signed, completed, and submitted to the NIDEP WITHIN 7 DAYS OF THE UST REMOVAL. This is the client’s responsibility; T. Slack cannot sign off on this questionnaire. TSHS will prepare and submit a full Site Investigation Report (SIR) and or a Remedial Investigation Report (RIR) as required by the NIDEP. The complete report with electronic data will be sent to the client for original signature by the responsible party. IT IS THE CLIENT’S RESPONSIBILITY TO SUBMIT THE REPORT TO THE NJDEP. Failure to do so will result in possible penalties issued by the NIDEP (See review fee below).
