Name of Applicant/Owner: _Mavis Tire/4up! , ihc 2/ tof Le Signature of Applicant/Owner: \ | | | David Sorbaro, Co-CEO This stormwater is classified: (Circle) Minor Development¢ Major Development Minor Development Definition: Any developments that result in a new dwelling and/or the construction of any site improvements that result in an increase in impervious cover by 400 square feet that results in an increase of stormwater runoff but does not meet the definition of major development; Major Development Definition: An individual development, including redevelopment as well as multiple developments that individually or collectively result in: 1. The disturbance of 0.5 acre of land since February 2, 2004; 2. The creation of 5,000 square feet or more of regulated impervious surface since February 2, 2004. 3. The creation of more than 5,000 square feet or more of regulated motor vehicle surface since March 2, 2021, or; 4. Acombination of #2 and #3 above that totals an area of 5,000 square feet or more. The same surface shall not be counted twice when determining if the combination area equals one-quarter acre or more. Major development includes all developments that are part of a common plan of development or sale (for example, phased residential development) that collectively or individually meet any one or more of paragraphs 1, 2, 3, or 4 above.
